We have case mangers on our staff who help frail, homebound seniors get connected with services they ned to help them stay safely in their homes. Thsi program helps stabilize the neighborhood, keep seniors healthy and happier for as long as possible, and reduces the costs of elder care on our community.

Each senior who is part of the program participates in a comprehensive assesssment meant to discover the person’s unmet needs.Some of the services we help people connect with are:

• Meals on Wheels
• Home care
• light housekeeping
• medicaid enrollment
• enrollment in other entitlement programs
• similar services
